About Support coordination

A plan is only as useful as what it actually connects you to. Support coordination is the work of turning a document into the services, appointments and relationships that make up your week.

Your coordinator helps you understand what is funded, weigh up your options, set supports up and keep them accountable. Where something is not working, they help you change it.

Coordination also means being ready for the next plan review, with a clear record of what worked, what did not, and what you want next.
